Improving Custom Surgical Trials: Total Talus Replacement

Our prototype, the Hex Handle, consists of a bullet tip, which allows the handle to easily slide into the grooves of the receiver, six identical prongs that allow for insertion and removal from any direction, and a receiver which allows for free rotation while inserted, but has divots for a rod to fit into, locking rotation.

Project Description: 

Osteoarthritis affects 1 in 10 people in the US and is one of the leading factors to reduced mobility in old age. The most debilitating cases are found in the ankle where it can induce avascular necrosis, referred to as AVN, causing the joint to collapse. Current treatment options for AVN include ankle fusion and foot amputation, both of which decrease patient quality of life. Total Talus Replacement is an alternative surgery where the surgeon removes the Talus and replaces it with a patient-specific cobalt chromium implant. Before final insertion, 3D printed trials of varying sizes are inserted to determine the appropriate final implant size. The issue with current trial devices is the handle impinges on the bones surrounding the talar space, which makes it difficult to assess proper joint articulation. Therefore, surgeons performing Total Talus Replacement surgeries need a better way of assessing fit, movement, and articulation before implantation.
Our solution, the Hex Handle, consists of a bullet tip, which allows the handle to easily slide into the grooves of the receiver, six identical prongs that allow for insertion and removal from any direction, and a receiver which allows for free rotation while inserted, but has divots for a rod to fit into, locking rotation. Combining all these, the hex handle allows for simple insertion and removal with maximum dexterity while locked into place.
This improvement to Total Talus Replacement surgery makes it more desirable for surgeons, as it cuts down on OR time and decreases incidence of improper fit. Due to the ease of manufacturing and the simplicity of its design, this solution presents a huge opportunity to expand to other Orthopedic implants beyond the realm of Total Talus Replacements. By ensuring the best fit of custom implants, the hex handle improves comfort, mobility, and quality of life.
